Abstract
Introduction : Lidocaine from the amide group is the type of local anesthetic that is most widely used in dental clinics because of its lower toxicity than other types of local anesthetics. Lidocaine has the potential to cause vasodilation when injected into the mucosa and skin, so the use of epinephrine is necessary. The use of epinephrine in local anesthetic 2% lidocaine in the extraction procedure has an important role in accelerating the onset of action and also increasing the duration of action of the local anesthetic solution. On the other hand, the use of local anesthetic 2% lidocaine without epinephrine will cause a shorter duration of action of anesthesia and uncontrolled bleeding which will complicate the extraction process. Research Method : The research design used was a literature review and a search for published articles was carried out on Google Scholar, Pubmed, Science Direct and Research Gate using predetermined keywords, namely local anesthesia, lidocaine, epinephrine and hypertension. This literature review uses articles published in 2010-2020 which can be accessed in full in pdf format. The article search criteria were in accordance with the inclusion criteria, namely research articles using hypertensive subjects aged 18-75 years, controlled hypertensive patients with systolic blood pressure of 120-159 and diastolic blood pressure of 80-99 mmHg, hypertensive subjects without systemic complications and not having an allergy to lidocaine, articles published in 2010-2020, and articles in Indonesian and English. And it is supported by several articles that do not meet the criteria but the available information supports the existence of this literature which is referred to as exclusion criteria, namely research articles that use hypertensive subjects with systemic disease complications, hypertensive subjects who perform extractions using local anesthetics other than 2% lidocaine. Conclusion : There was no effect on the use of epinephrine in the local anesthetic 2% lidocaine used in the extraction procedure on the blood pressure of pre-hypertensive and grade I hypertension patients.
