Abstract

The management of hazardous medical waste in hospitals is a strategic issue with direct implications for public health and environmental sustainability. In 2024, only 66% of healthcare facilities in Indonesia managed waste in accordance with standards. Karanganyar Regional General Hospital, a Type C hospital with high service activity, has the potential to generate large amounts of hazardous medical waste; however, its policy implementation has not been specifically examined from a policy perspective. This study employed a descriptive qualitative approach with a case study design. Data were collected through in-depth interviews, field observations, and document reviews with five purposively selected informants. Data were analyzed using the Miles and Huberman model, with source triangulation for validation. Findings indicate that hazardous medical waste management at Karanganyar Regional General Hospital generally aligned with Ministry of Environment and Forestry Regulation No. P.56/Menlhk-Setjen/2015, covering reduction, segregation, containment, collection, and processing through licensed third parties. Nonetheless, several gaps persist, including the absence of temperature recording at the waste transfer station (WTS), reliance on general pathways for internal transport, and uneven distribution of training and Hepatitis B immunization among staff. Policy implementation meets basic operational standards but remains suboptimal in technical execution and resource allocation.Recommendations include strengthening monitoring at waste transfer stations, establishing dedicated transport routes, ensuring equitable Hepatitis B immunization across employment categories, and expanding training frequency and coverage for all staff. These findings are expected to provide input for hospital management and local government in enhancing sustainable hazardous medical waste management.
