Abstract

Klinik Rumah Terapi Pelangi Insan is a child development clinic located in Banjarnegara Regency. The clinic focuses on observing the therapeutic needs required to support the growth and development of children. One of the problems faced by Klinik Rumah Terapi Pelangi Insan is the process of recording and observation, where the clinic still uses a manual bookkeeping system that’s not computerized. This metod is considered slow and hampers the patient service process, leading to delays in patient care and a risk of losing medical record data. This study aims to develop an information system for medical records and the observation of therapeutic needs at the child development clinic. The information system at the clinic includes patient registration data, observation of therapeutic needs, and patient medical records. An organized database allows clinic staff to access patient information quickly and accurately, as well as assist in determining the appropriate therapeutic needs based on observations and clinical data. The development method used in this system is the structured Waterfall model, which involves several stages: analysis, design, implementation, testing, and system maintenance. The programming language used is PHP with the Laravel framework, and MySQL is used for the database. The results of system testing show that the medical record data entry and therapeutic needs observation forms are effective in helping therapists store data and conduct therapy needs observations, with very good results accepted by therapists, achieving an average User Acceptance Testing (UAT) score of 92%.
