Abstract
Background : Nursing management plays an important role in ensuring quality of service and patient safety through the implementation of organized and standardized nursing care. One crucial aspect of nursing management is the implementation of handover as an effort to maintain continuity of nursing care between shifts. The ISBAR method is a structured communication standard recommended to improve clarity and accuracy of information during handover. However, the implementation of ISBAR at PKU Muhammadiyah Sampangan Hospital has not been optimal and consistent with the ISBAR format, so ISBAR training is needed to improve nurses' knowledge, skills, and competence. Objective : The objective of this study was to analyze the effect of ISBAR training on nurses' knowledge and skills. Method : This study is a quantitative study with a comparative descriptive design using a quasi-experimental method with a one group pretest–posttest approach. Nurses' knowledge was measured before and after ISBAR training, while skills were assessed through observation of ISBAR implementation during shift handovers 5 times for each respondent, resulting in 45 observations. The study was conducted at PKU Muhammadiyah Sampangan Hospital. Knowledge assessment was conducted on June 17, 2025, while skills assessment was conducted from June 18 to July 14, 2025. The research population consisted of 60 nurses using convenience sampling, comprising 34 nurses for knowledge assessment and 9 shift leaders who had previously attended training for skills assessment. The research instruments were a 15-question multiple-choice knowledge questionnaire and an ISBAR application skill observation sheet. Results : The results showed that ISBAR method training had a significant effect on improving nurses' knowledge and skills. The average knowledge score of nurses increased from a pre-test mean of 9.38 to a post-test mean of 12.88. The paired sample t-test showed a significant difference between the pre-test and post-test knowledge scores of nurses after ISBAR method training (t = −9.753; p = 0.001). The general picture of the ISBAR application skill observation results during handover showed that the Situation, Assessment, and Recommendation components had been applied more consistently, but the Introduction and Background components were still inconsistent. Conclusion : These findings indicate that ISBAR method training has a positive effect on improving nurses' knowledge and skills in performing handover, even though some ISBAR components have not been optimally applied. Therefore, this training supports the strengthening of nursing management and patient safety.
