Abstract

The era of globalization requires every industrial sector to survive the competition by continuous improvement. PT. Pertamina Persero Refinery Unit VI is one of the manufacturing companies with refined petroleum products that continues to make continuous improvement. One of the efforts of PT. Pertamina RU VI Balongan in continuous improvement is evaluating the supply chain process, because the company strives to meet consumer demand on time. Supply chain is a series of processes to obtain raw materials and processed, then can be taken seriously to consumers through the distribution system. Pt. Pertamina RU VI Balongan processes many types of products. However, the main product is Pertamax. In the process of distributing this product both from raw materials and finished products there are some obstacles that occur. Grey Failure Mode and Effect Analysis (Grey FMEA) and Fault Tree Analysis (FTA) are methods that can be used to identify constraints and find the root of existing problems that are next to find alternative solutions. Grey FMEA is a method to identify risks that occur by combining grey theory with FMEA. While FTA is a tool to identify the root cause of an obstacle with a top event approach. In Grey FMEA's approach in identifying the risk of Pertamax supply chain, 13 risks were obtained with selected constraints, namely the availability of RU VI Balongan stock and inadequate inventory because it has the smallest degree of relationship of 0.488. After that, with the Fault Tree Analysis approach, the root cause of the problem is bad weather, unfocused workers, sluggish in work, inadequate maintenance equipment, maintenance is not in accordance with SOP, and the company improves the completeness of infrastructure facilities.
