Abstract
Indonesia is a developing country that has high morbidity and mortality rates due to infectious diseases, one of which is pneumonia. Irrational use of 2 antibiotics can adversely affect patients and related health services. In Indonesia, the government issued PMK No. 8 of 2015, namely the Antimicrobial Resistance Control Program (PPRA). This study aims to describe the use of antibiotics, determine the rationality of antibiotic use using the Gyssens method, and determine the relationship between the rationality of antibiotic use on the therapeutic outcome of pediatric pneumonia patients at RSUD Dr. Moewardi Surakarta with PPRA and RSI Klaten without PPRA at the same time. This study is a cross sectional study with retrospective data collection from patient medical records. The inclusion criteria were patients aged 0-18 years with a diagnosis of pneumonia and receiving antibiotic therapy in hospitalization during the period January 2017 to May 2018, complete medical record data at least included patient identity (gender, age, weight or height), disease diagnosis , drugs given, temperature, respiratory rate, laboratory tests. The analysis was conducted descriptively and analytically with the Chi-square method to see the relationship between rational antibiotic use and therapeutic outcomes. The single antibiotic that is commonly used in RSUD Dr. Moewardi Surakarta and RSI Klaten is Ampicillin. In addition, antibiotics that are often prescribed are the third generation cephalosporins (Ceftriaxone and Cefotaxime). The use of antibiotics in Dr. Moewardi Hospital, there were 83 irrational use of antibiotics (81.4%) and 75 in Klaten Hospital, the number of antibiotics used (86.2%). Irrational category III A (long duration) occurred in two hospitals. The results of the research on the relationship between rationality and therapeutic outcome in the Chi-square analysis showed that the rationality of empirical antibiotic therapy increased the therapeutic outcome significantly (p = 0.008) in Dr. Moewardi General Hospital. The results of research conducted at RSI Klaten obtained p value = 0.153 which means that there is no significant relationship between rational use of antibiotics and therapeutic outcomes.
