Abstract

Service activities and data processing at Mediva Clinic Ngawi is still done manually and there is no clinical information system based on website. The research aim to make the design medical record information system design based on electronic according to the needs of users at Mediva Clinic Ngawi. The type of research is about Medical Record Information System Design based on electronic at Mediva Clinic Ngawi is research and development (R & D). The design of this research is using prototype method, the development of medical record information system at Mediva Ngawi Clinic. Data collecting technique is done by interview, documentation study and FGD to four informant as potential user of system to fulfill user requirement. The result of this research that has been made design of system design, there is a DAD level 0 for data core and data normalization at DAD level 1 is six according to user requirement. Database Design has been made, there are 11 database tables according to user needs. The design of the interface has been created and the prospective user chooses the design of the second choice interface and the menu has been in accordance with the needs of the user. Information system programming using XAMPP application, Notepad ++ and using PHP programming language. It is necessary to increase the facilities and infrastructure in Mediva Clinic Ngawi, need additional personal computer for each service. Need for additional human resources especially in patient registration service, because currently there is only one registration officer. Need for further research in terms of further development of clinical information system to be ready for use in Mediva Clinic Ngawi.
