Abstract
Hospital formulary is a list of drugs compiled by Tim Pharmacy and Therapy (TFT) and agreed to by the medical staff as well as referring to the national formulary, which are set and enforced by the leadership of the hospital. Nonconformity prescription drugs may result in reduced quality of hospital services and the cost of drugs used are not efficient. Minimum service standards for prescribing conform with the formulary that is 100%. This study aims to determine the percentage of adherence to the formulary drugs written in RSUI Yakssi Gemolong in the period from January to March, 2016. This research is a non-experimental, descriptive. Data were collected retrospectively is to observe and evaluate the prescription sheet drawn from the sample outpatient prescription sheet in January 2016-March 2016. Sampling was carried out at random sampling. Conformity measured by calculating the percentage of the number of prescription accordance with formulary and the number of all the prescriptions written by doctors for 3 months in the hospital. Results showed the percentage of the conformity of general outpatient with RSUI Yakssi Gemolong formulary in January 97,33 %, 96,79 %, in February, and March 96,26 %. The average percentage of conformity general outpatient prescription for 3 month of January 2016-March 2016 amounted 96,79 %.
