Abstract
Prescription review in prescribing phase is one of the pharmaceutical services with a view to improving the quality of life of the patient. The aim of this research is to know unsuitability in the prescribing phase in Surakarta pharmacy in 2017. The method is non experimental with retrospective sampel. The samples used were prescriptions of 50 prescriptions per month in April, May and June of 2017, and those analyzed were in administrative, pharmacological and clinical studies. This determination is based on Kepmenkes no. 129 tahun 2008 about Standar Pelayanan Minimal Rumah Sakit and PMK no 73 year 2016. The result of this research is unsuitability occurrence at prescribing phase 15,75%. In the administrative study the unsuitability occurred because: 15% unreadable prescription, no patient's weight and age in the prescription was 82,83% & 6,67% and number doctor's prescription was 80,1%. In the pharmacist study the unsuitability occurred because: 2% dosage form, 13,56% strength of drug preparation and drug incompatibility of 28,48%. In clinical study the unsuitability occurred because: error on the rules of use of 14,67%. 2% drug interaction occurred, excess dose 3,65% and less dose of 20,58%
