Abstract
Nutrition screening is a simple and quick process to identify individuals with nutritional problems and needs to be assessed for nutritional status by a medical expert. Assessment criteria are quick and easy with less than 10 minutes to complete. The goal of nutritional screening is to determine whether a patient is at risk of malnutrition or not. The SGA measurement method is a clinical assessment method. SGA is a nutritional assessment tool consisting of medical history and physical examination. This nutritional screening application is appropriate when applied in hospitals because the objects in this study do not yet have a widespread application or simply provide a conventional form. The design of this application is built with the method of Unified Software Development Process and application design using Unified Modeling Languague (UML). The workmanship mechanism of this research is made literally from the process of making Software Requirements Specification (SRS), then proceed with making Software Design , then followed by making application and closed by testing. The result of making this application in the form of SGA value. This nutritional skinning tool will automatically calculate the value that has been entered by the nutritionist and can be directly printed.
