Abstract
High rates of occupational accident indicates that there is still less of awareness of the labors either the company regarding health and safety work issue. Accident risk and occupational disease can be analyzed by hazard identification method. CV. Akurat has never identified factors and potential hazards. The aim of the research is to know the factors and potential hazard at the production part of CV. Akurat Mojolaban Sukoharjo. This research is qualitative research with critical incidents approach. The objects of the research are work process, human, tools and materials used during the production process. The result of identification shows that there are 38 hazard sources, 10 hazard sources in high hazard category, 7 hazard sources in medium hazard category, and 21 hazard sources in low hazard category. From the result of this research can be concluded that the cause of hazard source such as the unsafe working environment, the unsafe of equipment or machinery and materials, and the unsafe of working attitude. The suggestions for the company is to take control immediately toward hazard sources especially in high hazard and medium hazard level.
