Abstract
Occupational accidents remain a major concern in labor-intensive manufacturing industries, including small and medium-sized enterprises (SMEs), because they affect worker safety and productivity. This study aimed to identify workplace hazards, analyze the root causes of occupational accidents, and develop improvement recommendations at CV. Lumintu, a towel manufacturing SME. The research focused on the weaving, washing, dyeing, and embroidery workstations. Primary data were collected through observations, interviews, documentation, and literature review. Job Safety Analysis was used to identify hazards and assess risk levels, while Fault Tree Analysis was applied to determine the root causes of workplace accidents. The 6S concept was used to formulate workplace improvement recommendations. The results showed that the production process involved hazards with moderate, high, and extreme risk levels. FTA revealed that accidents were mainly caused by inadequate use of personal protective equipment, the absence of machine guards, unsafe working positions, low worker concentration, and poor workplace conditions. The 6S evaluation score was 2.68 (poor), indicating the need for risk control based on the hierarchy of controls integrated with the 6S concept to create a safer and more organized workplace.
