Abstract
Geographical accessibility to primary healthcare services is a key pillar in achieving Universal Health Coverage (UHC). Up to 2025, the high coverage of National Health Insurance (JKN) in Sukoharjo Regency has not been balanced by spatial studies mapping the relationship between healthcare facility distribution, population dynamics, and actual service coverage towards settlements. This study aims to analyze the spatial distribution patterns of Primary Healthcare Facilities (FKTP) and identify the physical accessibility level of these services to settlement areas in Sukoharjo Regency in 2025. This research is a descriptive quantitative study based on Geographic Information System (GIS) utilizing spatial and demographic secondary data from 2025. The research population includes all active FKTP units (Community Health Centers/Puskesmas, Auxiliary Puskesmas, Policlinics, Private Doctors, and Midwives practices). The data analysis methods used are Average Nearest Neighbor (ANN) to identify the general distribution pattern of facility points, Kernel Density Estimation (KDE) to map continuous density clusters (hotspots), and Buffer Analysis with a 3,000-meter radius to evaluate physical accessibility to residential polygons processed through spatial overlay techniques. The ANN analysis results showed a pattern discrepancy between government facilities (Puskesmas) which tend to be regularly dispersed across sub-districts, and private/independent FKTPs which significantly form a clustered pattern. The KDE interpolation map confirmed a high-level healthcare agglomeration (hotspots) in the northern urban corridor (Grogol, Kartasura, and Baki sub-districts), which is linear with population density. Conversely, the southern rural areas (Bulu and Weru sub-districts) exhibit low facility density values. Buffering results indicate that most northern settlements fall into the over-served category, while settlement pockets in the southern hilly terrain lie outside the ideal service radius. This spatial disparity is heavily influenced by geomorphological factors and economic agglomeration, where independent healthcare facilities naturally concentrate in alluvial urban growth centers. Climate factors, specifically high rainfall intensity in the northern zone (such as Mojolaban and Kartasura), periodically potentially decrease service coverage effectiveness due to travel time barriers. This study recommends that the local government formulate space-equity health zoning policies by deploying mobile health services or strengthening Auxiliary Puskesmas in southern underserved areas to eliminate blank spot zones.
