Abstract

Background: The high rates of mortality and disability associated with stroke make it one of the major global health problems, with a prevalence that continues to increase, particularly among high-risk populations with a combination of risk factors such as hypertension, diabetes mellitus, obesity, dyslipidemia, atrial fibrillation, smoking, and physical inactivity. Various primary and secondary prevention strategies have been developed to reduce the incidence and recurrence of stroke, however, their implementation in practice remains suboptimal. Purpose: This study aims to examine the most effective stroke prevention strategies for high-risk populations based on the most recent scientific evidance. Methods: A literature review was conducted using Randomized Controlled Trials (RCTs) and quasi-experimental studies published between 2015 and 2025. Articles were retrieved from PubMed, ScienceDirect, PEDro, and Google Scholar and systematically analyzed. Of 3,933 identified articles, seven studies met the inclusion criteria and were included in the review. Results: The reviewed studies indicated that targeted prevention programs improved health awareness, adherence to risk factor control, and healthy lifestyle behaviors in high-risk populations. Interventions incorporating education, digital support, and community or environmental involvement demonstrated positive effects on blood pressure control, physical activity, and lifestyle modification. Conclusion: Stroke prevention in high-risk populations is most effective through an integrated approach combining risk factor management, lifestyle modification, and continuous health monitoring. Structured and sustainable prevention strategies are required to optimally reduce stroke risk.
