Abstract

PT. Aneka Adhilogam Karya is a private company operating in the field of metal casting. In 2024, the company produced defective products at a rate of 5%–10%, exceeding the company's acceptable defect limit of 3%. Giboult joint is the most frequently produced product each month. The purpose of this study is to identify the causes of product defects at PT. Aneka Adhilogam Karya and to propose improvements for failure modes with the highest RPN (Risk Priority Number) value. The method used is Failure Mode and Effect Analysis (FMEA). Seven failure modes causing product defects were identified, with the highest RPN value of 270 found in the failure mode related to reduced moisture content in the sand mold due to the mold being left idle for too long before casting. Based on these findings, instructions for implementing a Kanban system were developed as an effort to schedule the production process more effectively and prevent delays in the casting process.
