Abstract
This research was conducted at UD Carissa Jaya Beton with a still high level of losses, because many defective paving press products were still found that did not meet standards. UD Carissa Jaya Beton received a customer complaint due to a defect rate of 600 press pavers out of a total of 6,500 pavers in a parking lot project. This condition resulted in the return of 24 square meters of material and financial losses. This research aims to identify the dominant types of defects, analyze the causal factors through Statistical Quality Control (SQC) and Failure Mode and Effect Analysis (FMEA), and formulate recommendations for improving the production process. The method used is SQC with the tools of P control map, pareto diagram, and fishbone diagram, and FMEA to determine the risk priority based on RPN value. Data were obtained through observations and interviews for one month. The analysis showed three dominant types of defects: rompal paving, cracked paving, and unsymmetrical paving, with an average defect rate of 4-10% per day, exceeding the tolerance limit of 2%. The main causal factors are human, machine, and material. Proposed improvements focused on process control and improving the quality of materials and machine maintenance.
