Abstract

Along with the development of various industrial sectors, the era of globalization has become one of the main problems in business competition which is increasingly growing rapidly. Quality is a fundamental strategic basis in producing goods, in order to win the competition, so companies need to improve product quality by carrying out quality control. PT. Kosoema Nanda Putra is a private textile company, this company is experiencing problems in decreasing the quality of the products produced, which is the cause of the decline in the quality of gray fabric products including various kinds of defects, namely crash defects, torn defects, non-woven defects, loose weft defects.and ring temple defects. To overcome these product quality problems, use two methods, namely Failure Mode And Effect Analysis (FMEA) and Fault Tree Analysis (FTA). The aim of the research is to identify the highest defects and the causes of defects using the Fishbone Diagram tool. Calculate the highest RPN value using the Failure Mode And Effect Analysis (FMEA) method and determine proposed improvements using the Fault Tree Analysis (FTA) method. Based on the analysis of gray cloth defects from the production process, the highest type of defect, namely crash defects, was 17,328m during the last year, 4 highest causal factors were identified, namely decreased machine performance by 19%, incorrect machine parameter settings by 15%, spare parts wear by 12%, and operators are less than 10% careful so improvements are needed to reduce the defect rate.
