Medical record is a file that contains notes and documents about patients containing identity, examination, treatment, and other medical actions which are managed either publicly or privately. Medical records contain facts related to a case (strengthened by the existence of a visum et repertum). This is because medical records can be a means of proving cases. Evidence is an attempt to find the truth in a case and to present a general picture that further persuades the court in forming a verdict and decision about the truth and in assessing the guilt of the plaintiffs.
