Abstract
PT Djarum Kudus is one of the cigarettes producing companies where one of the most requested SKT cigarette products is Djarum Coklat. The purpose of this study is to determine the type of product defect, evaluate the factors causing dominant defects, and provide improvement proposals to improve quality. The number of product failures produced by the company so that the company must make an improvement in quality by using a method to reduce the number of defective products that occur. In an effort to reduce the number of defective products by controlling the quality of the product, the methods used are the Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA) methods. This research resulted in 3 dominant defects, untidy talipan of cigarette, rings menceng > 1 mm, and medot. The cause of product failure tends to be caused by employees who are less careful and hasty in working, and the limited tools supporting cigarette production on the production table. So that the proposed improvements used are the addition of milling and batil workers to meet high production targets, improving the performance of supervisors and QC Inline, regular maintenance of milling machines, and the addition of supporting tools for cigarette production such as magazine tools.
