Abstract
Periodontitis is a multifactorial complex inflammatory disease that causes damage. Inflammation of periodontal tissue in cases of chronic periodontitis increases the macrophage phenotype ratio of M1/M2, expression of IL-1β and MMP and macrophage migration inhibitory factor. Polymorphonuclear neutrophils (PMN) also experienced an increase in chronic periodontitis followed by an increase in apoptosis and degranulation. Objective: To determine the role of macrophages and neutrophils in severe periodontitis. Methode: This literature review uses a type of Narrative Review research with secondary data related to the role of macrophages and neutrophils in severe periodontitis. Article searches were conducted from December 2021 to January 2022 through several search engines, namely (1) PubMed, (2) Google Scholar, (3) Wiley, (4) Sciencedirect and (5) Springerlink. The keywords used in the search process are (Roles for OR Expression) AND (Macrophage OR Neutrophil) (Periodontitis Stage III OR Severe Periodontitis). Discussion: An increase of macrophages M1 and neutrophils in severe periodontitis. Activation of macrophages and neutrophils plays a role in phagocytics of red-complex bacteria that colonize, causing deepening of the sulcus to pocket formation. Macrophages release pro-inflammatory cytokines, PGE2 and MMP, while neutrophils produce enzymes that are toxic to bacteria. Macrophages also play a role in the process of tissue repair and angiogenesis by inducing growth factors. Conclusion: The results of comparison among several literature show that macrophages M1 and neutrophils increases in severe periodontitis, macrophages (M2) also play a role in tissue repair and angiogenesis.
