Abstract

PT. Sumber Inti Pangan is a manufacturing company engaged in the food manufacturing industry. One of the products is Hot Pasta & Spicy Sauce. In this product often found defect on packaging in the form of leakage, gramasi, and not appropriate size. The research aims to identify potential failure modes with the failure Mode and Effect Analysis (FMEA) method and the cause of the failure mode with the Fault Tree Analysis (FTA) method as well as provide improvement proposals to the Hot pasta production process & Spicy Sauce. Identify the cause of the dominant defect using Fishbone diagram. Further analysis uses the FMEA method to determine the most potentially failure mode and the FTA method to analyze the root cause of the failure mode. Defect is the most dominant leak of 91%. There are 5 potential failure modes that cause defect i.e. there is no standard viscosity paste, less homogeneous pasta, unfocused operator, timing of filling on improper machine and no maintenance preventive. By using FTA known there are 14 factors that cause potential failure mode to occur, resulting in 14 repair solutions to minimize the occurrence of leaks in the product packaging.
