Abstract

CV. Laksana karoseri is one of the body bus companies in Indonesia. Kaksana has a main division and several small divisions, one of the smaller divisions is the vacuum division, the vacuum division Producing bus interiors, the vacuum division uses two types of machines, namely CNC Thermoforming T10 and CNC Milling FZ2000 machines. In operating the machine, machine failure often occurs, causing downtime. This study aims to analyze the failure factors on the CNC-1 and CNC-2 machines. The analysis was carried out by conducting interviews to determine the type of damage and potential failures that occurred. Based on the research, it was found that the failure on the CNC-1 machine had a top failure event, namely valve break with Severity of 7, Occurance of 2, Detection of 4 with an RPN value of 56 and the CNC-2 machine had a failure top event, namely a worn drill bit with a Severity of 4, Occurance of 7, Detection of 3 with an RPN value of 84 the highest RPN value. The results of the analysis of the causes of damage to CNC-1 caused by not replacing parts regularly, lack of maintenance carried out, while the cause of CNC-2 is the occurrence of operator errors when setting up and using excessive drill bits. The improvement proposal given is to prioritize repairs in the event of a failure that has the highest RPN value for each failure that occurs.
