The sampling technique used is purposive sampling. Instruments used to record data using a questionnaire, sphygmomanometer, and stethoscope. The analysis used is Chi-Square. The results of this study are based on the Barthel index score, most DM patients have mild dependence and obtained a p-value of 0.034 (<0.05) which means that Ho is rejected so that there is a relationship between daily living activity and the incidence of peripheral artery disease diabetes in patients with diabetes mellitus. at the Sukoharjo Health Center with a correlation of -0.220, which means the lower the daily living activity level, the higher the risk of peripheral artery disease
