Abstract

The registration information system and clinical medical records or public health centers is an information system used to record patients and their health medical records. Information systems are needed to minimize procedural errors in the implementation of registration and management of patient data. The information system is designed to make it easier for health workers to manage computerized patient data. This information system for registration and medical records of clinics or health centers contains patient information data and computerized patient medical record data, where previously the performance of the system in data collection of patients at health clinics in general was not optimal and still uses manual processing of patient data using bookkeeping media. This method is considered less effective and efficient in data collection of patients. This website-based clinical medical record and registration information system is designed using the Waterfall method and the PHP programming language supported by a MySQL database, with data collection techniques including observation and interview methods. In this system development research, unit testing of the system is also carried out using black box testing and SUS testing to test the system whether the system is suitable for use and whether the system has errors or not. The black box test shows that the functionality of the application runs without problems with 100% success, while from the SUS test obtained from several respondents, it can be concluded that the SUS test is in the best imaginable category with a scale of > 80 to > 90.
