Abstract

Residual Schizophrenia is a type of schizophrenia with a record of at least one psychotic episode, followed by more prominent negative symptoms. Antipsychotic use is the mainstay of therapy, but long-term use of antipsychotics predisposes to problems such as adverse drug reactions, resulting in decreased adherence, relapse, and poorer quality of life. This study aims to identify the description of the side effects of antipsychotic drugs, identify the level of quality of life on the use of antipsychotics, and evaluate the effect of the emergence of ADRs on the quality of life of residual schizophrenia patients in the outpatient installation of at dr. Tjitrowardojo hospital, Purworejo Central Java from October to November 2020. Data collection was carried out through direct interviews using the Naranjo Scale, Prince Henry Akathisia Scale, Insomnia Severity Index Scale, and the EQ5D questionnaire. Data analysis used univariate and bivariate analysis with SPSS version 16.0 program. The sample of this study were patients who adhered to medication, aged > 18 years, outpatients diagnosed with residual schizophrenia for at least 1 month of treatment, had normal laboratory test results, and were willing to fill out questionnaires while the exclusion criteria were incomplete medical record data, receiving metoclopramide therapy. within 1 month, taking high doses of antipsychotics, and having a history of Parkinson's disease, stroke, diabetes mellitus, and head injury. This study obtained a sample of 97 patients. Most of the patients received treatment with Haloperidol + Clozapine 37.1%, Risperidone + Clozapine 10.3%, and Clozapine 10.3%. The highest side effect cases were on the use of Haloperidol with tremor 36 (24.66%), rigidity 16 (10.96%), akathisia 15 (10.27%), and bradykinesia 6 (4.411%), weight gain 11 (7, 53%), tardive dyskinesia 2 (1.37%), acute dystonia 2 (1.37%), and erectile dysfunction 1 (0.68%). The use of a single antipsychotic had a significant relationship on the patient's quality of life with p<0.05. There was a significant effect on the incidence of ADR with symptoms of extrapyramidal syndrome with quality of life with p<0.05. From the study of side effects and quality of life of schizophrenic patients that need to be considered in the selection of antipsychotic drugs. Pharmacists can contribute to monitoring drugs related to the onset of effects, both extrapyramidal and non-extrapyramidal symptoms, so that they can produce side effects and achieve effectiveness in therapeutic success.
