Ischemic stroke is caused by the blockage that triggers the decrease of bloodstream rate to the brain. Drug related problems contribute to the level of morbidity, mortality, and the decrease of patient’s life quality. The purpose of this research is to study about drug related problems untreated indication, medication use without indication, underdose, overdose, drug interaction categories in hospitalized ischemic strokepatients at RSUD Ir. Soekarno Sukoharjo period 2017. This research is a non-experimental research with retrospective method with purposive sampling and the criterias are patient diagnosed ischemic stroke, hospitalized in RSUD Ir. Soekarno Sukoharjo period 2017, medic record number, patient’s age, patient’s weight, profiling blood glucose, profiling lipid, patient’s therapies include medicine, dose, frequency, and the way applying a medicine. The data is analyzed descriptively. The analysis was carried out based on 2011 Stroke Guideline by PERDOSSI and the guideline by AHA/ASA namely Guidelines for the Early Management of Patients With Acute Ischemic Stroke, Guidelines for the Prevention of Stroke in Patients With Stroke or Transient Ischemic Attacks, Stockley’s Drug Interaction 8th Edition, Drug Information Handbook 17th, and https://www.drugs.com/drug_interactions.html . From 60 patients who were examined, it was found that the most drugs prescriptions were on 98,33% patients using cerebral vasodilator citicolin and 96,67% patient using antiplatelet. From 358 prescriptions, there were 130 cases of drug related problems consisting of 37 cases of untreated indications (28,46%), 1 medication use without indication (0,77%), 27 cases of underdose (20,77%), and 65 cases of drugs interactions (50,00%).
