Abstract

Pneumonia is lower respiratory tract infection caused by bacteria, viruses, or fungi, which often attacks toddlers and parents over 60 years. The main therapy is used antibiotics, improper use of antibiotics can lead to resistance where data antibiotic resistance has reached 700,000 people per year at the end of 2014. In the therapy given several drugs so vulnerable to drug interactions that can be dangerous. The purpose of this study was to evaluate the accuracy of the use of antibiotics as well as determine the incidence rate of potential drug interactions in geriatric community pneumonia patients at the inpatient installation of the RSUD Dr. Moewardi in 2017. This research is a non-experimental study. Sampling was done retrospectively with purposive sampling method and descriptive data presentation. The inclusion criteria for this study included geriatric patients (over 60 years) who were hospitalized with community pneumonia and received antibiotic therapy and patients with complete data. Data were analyzed based on the 2016 KDP guidelines, 17th edition Drug Information Handbook (DIH), Medscape.com, Drug Interaction Fact, and Stockley’s Drug Interaction 8th edition. The results showed 53 patients who fulfilled the inclusion, got the right indication of 100%, the right patient 100%, the right medicine 56.14% in 57 cases, and the exact dose of 73.68% in 76 cases. Drug interaction potential at 594 prescriptions is 15.15%, there are 90 cases of potential interactions divided into minor severity 54.44% (49 cases), moderate 33.33% (30 cases), and major 12.22% (11 cases ) On the mechanism of pharmacology, pharmacokinetic interactions 42.22% (38 cases) pharmacodynamics 18.89% (17 cases) and unknown 38.89% (35 cases). Major interactions often cause serious effects, so drug combinations are avoided.
