Dinas kesehatan kabupaten Sragen oversees all puskesmas in sragen district. Every day the Puskesmas records all data, suck as financial data, administrative data, and disease data. Disease data will be reported to the health office by the puskesmas as the results of reports collected from each region together and become reports for all Sragen district health centers. The report will later become a report from Sragen district which was sent to dinas kesehatan provinsi jawa tengah. The main problem in sending data from the puskesmas to the health office is the lack of discipline of the reporting officers from the puskesmas who are not on time in reporting the data. Like data from the field of pengendalian dan pemberantasan penyakit (P2P), every month the P2P field receives reports of disease data from all health centers in Sragen district. Reporting carried out is still using e-mail, where the reports received are not neatly arranged and less structured due to being crushed by other e-mails. Frequently experiencing delays in sending data carried out by officers at the health center causes the processing of data in the P2P field to be hampered. Based on these problems the solution to overcome these various problems is by applying a data reporting and monitoring system. This system aims to facilitate health centers and health offices to manage P2P field report data. The system is made with the waterfall method where there are stages such as problem identification, design analysis, implementation, testing and conclusions. The system is made with CodeIgniter framework and as a support database using MySQL. The stage of data collection in making this website is by interview and observation methods. Website-based system and can be accessed through personal computer devices during internet connection. The results of the research test through the questionnaire state that the average response of system users is 94.2% with very good indicators. This system is expected to solve problems that occur in the P2P field in the dinas kesehatan kabupaten Sragen.
